Employers are required by the OSHA Act of 1970 to report any incidents resulting in the death of any employee and any incidents resulting in the in-patient hospitalization of three or more employees. Fatalities caused by heart-attack at work must also be reported to OSHA. The Mine Safety and Health Act requires reporting of each of the following cases; death at a mine, injury that may cause death, entrapment of an employee for more than 30 minutes with a potential to cause death, accidental ignition of gas, and any unplanned fires within mines.
