While doing all the analysis on the data I compiled from a community which I did a family assessment on. Here, I have created an action plan which will provide an analysis of the role played by public/community health nurse in the community and also analyze the influence of available social determinants of health care in the community. It is essential to appreciate the role of community/public health nurse in any community as well as my clinical family. Community improvement can be attained through data analysis based on these two factors. The importance of community/public health nursing can be illustrated like in the case P. Anne who did an empirical study on how to improve the health of communities’ nurses serves through education of society members on the disease prevention and safety practices, physical wellness and nutrition. While analyzing my clinical family, I realized that the family live in a more diverse community where the role of the community health nurse is paramount.
Several social determinants influence the quality of public health care offered in this community. The main social determinant that I noticed in this community is inadequacy in the transport system. Lack of proper transportation in this community is a social determinant in that it affects the accessibility of quality health care of the community members. Previously in the community assessment task, I outlined the structural set up of this community where I put certain issues such as security, transport, availability of schools, availability of eateries, and availability of recreational facilities into considerations. Even though social amenities in this community are accessible in a car drive, individuals who don’t own such private vehicles encounter challenges while walking to search for health care and other valuable products such as grocery goods. Another division in this community is created by the difference in the levels of incomes where the rich dominate the community. This is evidenced by the fact that many private vehicles affect the effectiveness of public transport. Here, I will now focus on how to develop my health education action plan and establish measures to mitigate the identified social determinant influencing public health.
In this study, I will use the healthy people 2020 Leading Health Indicator (LHI) topic, which is directly related to how my participating family’s community to access health care services. As I had previously discussed, there is a transport system such as train and buses although their terminal is at a far walking distance. This implies that it is quite difficult for individuals to access facilities while relying on the public transport system. From the fact that the data I collected shows that private vehicles are many, public transport such as buses work at loses and hence their time schedules of operations are not reliable in this community. By the use of public transport, members of this community suffer time loss as compared to the individuals using private vehicles. This condition in this community shows the biasness in access to health care services. Members relying on public transport have limited access to health as compared to individuals with private vehicles. The study eludes that inadequacies in transport are such a major issue that would influence public health, especially when considering the homeless and older people who cannot drive themselves.
In my case study, the family’s community access to health service needs particular nursing interventions to address the current issues of homelessness. In my study, I could identify three interventions which could be applied to benefit this community. These three services include; mobile health outreach, the establishment of primary clinic care units in vulnerable areas, and development of assertive community treatment program. For the implementation of these three measures, collaborations in the provision of quality health care to the community need to emphasize. These collaborations would involve engagement of government and non-governmental organizations that have concern for the community health. Plans to develop more clinical facilities in these areas would much help lower the biasness evidenced in the access to health care facilities due to inadequacy in transport. Empowering public health nurses would help them educate the community in the available disease prevention measures and the importance of quality nutrition. It is essential to conclude that, the roles of community/public health nurse cannot be ignored in this community set up, and there is a need to empower them.
