Nurses must be aware of several factors that affect the coordination of care and a continuum of care that effectively uses resources in the community and is part of an ethical framework that ensures professionalism in nursing.

Nurses need to collaborate with the patients and their family members to achieve desired health outcomes. This means working with the patient and families as partners in the provision of health. In this coordinating way, the patient defines who their family members are and their involvement in health care and decision making. (Swartwout,2016). Hospitals ambulatory practices and health care systems integrate this approach of patient and family-centered approaches into their daily operations to ensure a continuum of care. There are several ways of ensuring coordination between the patient and their families and other care providers; the patient and the families are encouraged and supported as essential members of the health care team across the continuum of care. Patients and families are also supported and encouraged to be involved in collaborative self-management support. They set goals and action plans, especially in the management of chronic illnesses in ambulatory settings. The family member is viewed as allies for the quality and safety in the health care setting, at the bedside, and even in the community. Care provided by the nurses is also offered according to the patient and their family members’ preferences. In the nursing change of shift, the report is done at the patient’s bedside and the family. The patients and family members are involved in the discharge planning and the transition phase of the patients. During the provision of care, the nurse will appropriately document preferences, observations, and reactions of the patient and the family members in the continuum of care.

An example of patient and family-centered coordination is during the discharge of the elderly with diabetes. The nurses must educate the client and their family members to ensure cost-effective plans and a positive outcome. The patient and the family members are educated on insulin administration for the patient.they are given information on the dosage of insulin, storage conditions, and administration of insulin to prevent high insulin levels, which is a precursor for diabetic complications.

Change management is a structured approach to changing individuals, teams, and the organization from the status quo to desired future states.in health, it focuses on quality improvement to ensure the patients’ satisfaction and the organization. Change management involves the improvement of health services by improving medical education. Practical and clinical systems that are effective require leaders committed to safety and reliability.this may necessitate new criteria in the recruitment of nurses and other healthcare workers.promotion is also vital in ensuring quality patient-centered care. Medical educators think based on preparing future nurses and doctors that are efficient and effective inpatient care. Strategic improvement is based on adopting a systematic approach to ensure the realignment of care. Change management ensures patients trust the nurses and the doctors, increasing their satisfaction with the care provided. Influencing patients’ care outcomes is based on several forces that must be considered in a more ethical decision. The forces include a good relationship with patients, knowledge and skills, and an understanding of the relevant ethical principles. Ethical decision-making is an essential factor in the coordination of patient care. Information sharing with different parties requires high ethical standards, and the decision made is necessary to be highly confidential. Coordinated care should be based upon the ethics to which the community recognizes. The ethical approach to coordinated care will be essential in ensuring that the community supports the healthcare professionals’ standards and regulations and support their activities within the given jurisdiction. Decision making is guided by several ethical principles, including; utilitarianism and deontology. Deontology implies when ethical decisions are made when individuals remain to their roles and obligations while making decisions. Utilitarianism coordinated nurse will make decisions based on being fair without considering the outcome.

Health insurance pays the medical expenses of the patient. The patients can pay for insurance through personal means or through government initiatives that cover specific care. The health insurance policy has provisions that protect different patients. Every patient has various experiences in the health insurance policy. Others have bad experiences with the health insurance policy, especially when the insurance firms fail to deliver and do not pay for their medical expertise. The health insurance cover helps to diminish financial constraints to the patients. It ensures the clients to get services when needed. It also ensures continuity of care and coverage for some clients. Its limitation is the wide variability in how health insurance is measured and defined. The premium for health coverage increases with age. This affects the older patients as most do not have any income source due to retirement.so.sometimes, insurance is limited to what it can cover. This affects the health outcome of individuals.
